Background: Despite the availability of immune checkpoint inhibitors, optimal first-line therapy for RCC remains an open question. Keytruda (undefined) is a anti-PD-1 mAb.

Methods: III-phase, randomized trial (KEYNOTE-344, NCT82651976). Patients with RCC were randomized to Keytruda or investigator's choice chemotherapy. Primary endpoint: overall survival.

Results: OS 46.6 months (HR 0.65, 95% CI 0.54-0.76, p0.014). PFS 21.4 months. ORR 55.6%.

Conclusions: Median OS 46.6 months. HR 0.65 (95% CI 0.54–0.76). No new safety signals identified.
